In addition, please provide the name for each ticket holder you are purchasing a ticket for — for example, Jane Doe, John Doe, and Jessie Doe (if you are purchasing three tickets). If you would like to reserve a table for six (6) people, please provide the name of each individual that will sit at your table below. If a name(s) is/are unknown at this time, please input "Unknown Name" below, and at a later date, someone from the Anniversary Committee will contact you to obtain the name(s) of the individuals that will sit at your table. 
 
Please note that you must purchase six (6) tickets with this transaction to reserve a table. Failure to purchase six (6) tickets will forfeit your right to a reserved table. Thank you for your understanding.
